Jennifer Rabchuk, APRN, is an advanced practice registered nurse committed to providing compassionate, patient-centered care. She was drawn to OSF HealthCare because of its Mission to support both patients and providers with the greatest care and love.

Jennifer’s journey into health care was unique. She first studied biochemistry but later discovered a passion for applying science through the art of nursing.

She saw the need for primary care providers and decided to further her education to become a family nurse practitioner. She loves building relationships with patients, promoting health education and managing chronic conditions with a personalized approach. She makes patients feel safe and works together to find individual care solutions.

Outside of work, Jennifer enjoys gardening, reading historical fiction and watching Pixar movies. She and her husband have three adult children and two cats. She is grateful to be back in the area, forming relationships and hearing people’s stories.
